Donation Request

Thank you for your continued patronage to Hair Excellence Salon & Spa.

 

It is an important part of our mission to give back to the many people and communities that have supported our growth and success. Our donation budget is planned on an annual basis and while we feel each and every charitable activity is both valid and gracious, we receive several donation requests each year. Regretfully, we cannot contribute to every request we receive.

Please take a moment to review our donation policy, as we can only consider requests that meet the following guidelines:

• Requests for donations will not be accepted by phone.
• We ask that all donation requests be submitted at least 3 weeks prior to the date they will be needed. This will allow us ample opportunity to review the information and correspond as necessary.
• The event must be held within a 30 mile radius of the Sun Prairie community
• The request must be submitted on the Hair Excellence Salon & Spa donation request form. No other requests, written or otherwise, will be accepted for consideration.
• Please attach any additional literature regarding the event to the donation request form.
